Dysautonomia Checklist?

I'm looking for a printable dysautonomia checklist that is categorized by area (neurological, digestive, vision, endocrine, etc...) I was thinking for chronic issues there could be a check mark and for symptoms that occur only during flares a circle or x or something like that. Does this exist? If not, I'm going to create one myself. I just don't want to recreate the wheel!

Feel free to give suggestions of symptoms that I should be sure to include that you feel are commonly forgotten. I'd love for this to be useful for people to print out and take to doctors.

Jackie, I don't recall where I got this list from ...sorry I can't give proper credit, but I've modified the original version a lot in my 2 years of POTS.

Lightheadedness

Fainting or near fainting

Generalized weakness

Palpitations

Extreme fatigue

Sleep disorders

Headache/migraine/tension headache

Myofascial pain (muscle pain accompanied by trigger points)

Neuropathic pain burning or coldness, "pins and needles" numbness ,tingling, or itching sensations

Cognitive impairment (brain fog/cog fog)

Tremulousness

Shortness of breath

Chest pain and/or discomfort

Loss of sweating

Excessive sweating

Night sweats

Delayed gastric emptying

Bloating after meals

Nausea

Vomiting

Abdominal pain

Diarrhea

Constipation

Dysphagia (difficulty swallowing)

Bladder dysfunction (excessive urination)

Pupillary dysfunction

Blurred Vision

Tunnel vision

Dizziness (Vertigo)

Tachycardia

Exercise intolerance

Clamminess

Anxiety

Flushing

Postprandial hypotension (low blood pressure after meals)

Blood pooling in limbs

Intolerance to heat

Feeling cold all over

Low blood pressure upon standing

Cognitive impairment ( concentration, brain fog, memory and/or word recall)

Narrowing of upright pulse pressure

Cold hands and feet (and often nose)

Chills

High blood pressure

Hyperventilation

Low back pain

Aching neck and shoulders (coathanger pain)

Noise sensitivity

Light sensitivity

Disequilibrium/Imbalance

Arrhythmias (irregular heart beats)

Chemical sensitivities (can be very sensitive to medications - may only need small doses)

Easily over-stimulated

Feeling full quickly

Feeling "wired"

Food allergies/sensitivities (some foods seem to make symptoms worse)

Hyperreflexia

Irregular menstrual cycles

Loss of appetite

Loss of sex drive

Muscle aches

Joint pains

Swollen nodules/lymph nodes

Polydipsia (excessive thirst)

Weight loss or gain

Feeling detached from surroundings

Restless leg syndrome

Unsteady gait

Jaw Pain / Tension (lower face numbness)

Slurred speech and misplaced words

Popping in joints

Electrical surging sensations

Muscle twitches

Thanks for the ideas. I'm wanting to get this done before I go for 2 weeks of intense biofeedback treatment (POTS Treatment Center) July 20. I think it could really help me notice improvements I may otherwise overlook. When I get it done, I will post a link.

In the meantime, keep the ideas coming!
